ANNUAL EVALUATION FORM
Supervisors will be responsible for the annual evaluation
of librarians in their major area of responsibility.
Librarians should submit this annual self evaluation form to their
supervisor to document
their yearly activities.
- Outreach
Represent the Library at campus or library sponsored activities,
departmental meetings, and/or campus committees.
- Bibliographic Instruction
Teach classes in library use, information literacy, and other topics.
(Include the number of annual instruction sessions.)
- Collection Development / Information Access
Selecting, evaluating, and monitoring print and electronic resources for the
Library.
- Reference
Provide information or reference assistance in person, by telephone, via
email, or through virtual reference.
- Department Evaluation and Development
Participate in evaluation and continuous improvement of the Library Liaison,
Bibliographic Instruction, and/or Reference programs as a whole.
